Immunizations:
A chart of the required immunizations for school entrance is available to view in the printable forms list. Your student will not be allowed to enter school without appropriate immunizations and dates given, an appointment for immunizations, or a completed exemption form.

Oral Medications: Because of changes in Washington State law in regards to administration of oral medication at school, the following rules now apply:

BOTH PRESCRIPTION AND NON-PRESCRIPTION MEDICATION (including Tylenol, ASA, Advil, Motrin, cough drops, cold medications, and other over-the-counter drugs) must be furnished to the school in the ORIGINAL container and must include the student’s name, name of the medication, amount to be given, along with the Oral Medication form (available below) signed by both the parent and physician. All medication should be ready to be administered and must not require any preparation by office staff.

Medicine may not be delivered by the student or bus driver. It is the parent’s responsibility to deliver and maintain and adequate supply. If the medication is delivered by the student or bus driver, the medication WILL NOT be dispensed and the parent will be notified.

Head Lice Information: Helpful information can be found here or you can contact your school nurse or family doctor with questions. Click here for the District Policy.

Student Medical Alerts/Food Allergies: Parents should contact the school nurse so that an appropriate care plan can be developed.
